Controlled Substances Prescription Monitoring Program Registration
A.R.S. § 36-2606 requires each medical practitioner who is licensed under Title 32 and who possesses a DEA registration to also possess a current controlled substances prescription monitoring program registration issued by the Board. There is no fee to the practitioner for this registration. This registration includes: MD, DO, DDS, DMD, DPM, HMD, PA, NP, ND, OD, and DVM.

Do I need to register?
Yes, every practitioner who has a DEA registration and is licensed under Title 32 must register.
Why do I need to be registered?
First, it is required by law. Second, as a registered medical practitioner you may have access to the controlled substances database giving you a useful tool to help you treat and evaluate your patients. If you choose to use the system, you must complete a database access request form. Once you receive your user name and password, you may access your patient’s prescription information from the database.
What does DISPENSE mean?
If a practitioner stocks Schedule II, III, and IV non-sample drugs and dispenses the drugs to patients or animals for use at home, then the practitioner will eventually be required to provide certain information about the dispensed drugs to the CSPMP on a regular basis.
Dispensing information related to samples or office administration of drugs is exempt from the reporting requirements.
If you are a dispensing practitioner subject to the reporting requirement, check the “yes” box on the form, and you will be contacted by the CSPMP to coordinate the collection of the information about the drugs dispensed.
If you are a practitioner who only writes prescriptions (prescribes), check the “no” box on the form. Remember that this does not affect your use of samples or any office administration of the drugs.
