Fees
New pharmacist application fees are $50 if applying by score transfer or full exam and $300 if applying by reciprocity.

Obtaining Your Pharmacy Intern License

Requirements
New Student Pharmacy Interns will require current transcripts or a letter from the registrar or dean certifying their enrollment and attendance at an approved college of pharmacy.

New Graduate Pharmacy Interns will require final transcripts from an approved college of pharmacy, showing the date of graduation.

New Foreign Pharmacy Interns will require a notarized copy of your FPGEC certificate.
Please do not send originals. You must be a U.S. citizen or legal resident with permission to work in the U.S.

PLEASE NOTE:

• Intern licenses are issued for five years. A sixth year can be requested and in most cases granted by the Executive Director; there is no need for a currently licensed intern to apply for a “graduate” intern license (they are one and the same).
• No intern certificates for immunization can be granted before July 20, 2011. Any applications received before that date will be placed on hold and processed on or after that date.

Fees
New intern application fees are $60 for one five-year license.
Intern licenses cannot be renewed or extended beyond the initial five-year license period.

Obtaining Your Pharmacy Technician License

Requirements
New Pharmacy Technician Trainees will require a copy of your U.S. Birth Certificate and High School diploma. (IF YOU ARE NOT A U.S. CITIZEN you MUST be able to provide proof of legal U.S. residency.

New Pharmacy Technicians will require a copy of your U.S. Birth Certificate and PTCB Certification.

Fees
New technician trainee license fees are $46 for a two-year license (includes license + wall certificate).

New technician license fees are prorated - please contact the Board for current fee.

Obtaining Your Immunization Registration

Requirements
New immunization applicants will require a current Arizona pharmacist or intern license, proof of completion of a training program specified in A.A.C. R4-23-411, and a current certificate in basic cardiopulmonary resusitation.

Renewal immunization applicants will require a current certificate in basic cardiopulmonary resusitation, as well as 5 hours of ACPE-approved immunization related CE.

PLEASE NOTE:

• Interns are not to give immunizations unless under the supervision of a licenced pharmacist who also holds a valid immunization certificate.

Requirements
New pharmacy intern preceptor applicants will require a current, unrestricted Arizona pharmacist license, and a period of licensure at least one year or greater.
