Requirements
New immunization applicants will require a current Arizona pharmacist or intern license, proof of completion of a training program specified in A.A.C. R4-23-411, and a current certificate in basic cardiopulmonary resusitation.

Renewal immunization applicants will require a current certificate in basic cardiopulmonary resusitation, as well as 2 hours of ACPE-approved immunization related CE.

 
PLEASE NOTE:
  • Interns are not to give immunizations unless under the supervision of a licenced pharmacist who also holds a valid immunization certificate.

IF YOUR LICENSE OR PERMIT IS EXPIRED, DO NOT APPLY FOR A NEW LICENSE OR PERMIT - THIS WILL ONLY DELAY YOUR RENEWAL.

Effective Monday, November 7th, 2011, renewals will only be payable by check, money order or cash (exact change) - no credit or debit cards.
