Requirements
New immunization applicants will require a current Arizona pharmacist or intern license, proof of completion of a training program specified in A.A.C. R4-23-411, and a current certificate in basic cardiopulmonary resusitation.
Renewal immunization applicants will require a current certificate in basic cardiopulmonary resusitation, as well as 2 hours of ACPE-approved immunization related CE. |

PLEASE NOTE:
- Interns are not to give immunizations unless under the supervision of a licenced pharmacist who also holds a valid immunization certificate.
